aPolicy without tiering. With no agreed routing test, every use case gets the tier-1 treatment. The queue becomes the control, and the control is indistinguishable from a stop.
bStandards nobody can run. A standard that is not a library, a template or a pipeline check is a document. Documents drift from the systems they describe within a quarter.
cEvidence on request. If assurance has to ask a delivery team for the eval results, the results are a development tool, not a control. The plumbing is what makes the difference.
